Some US families use backyard homes, sheds and trailers for aging relatives instead of nursing homes. Linda Barton and her husband paid $37,000 for a tiny home for her parents and $43,000 for one for her father-in-law; caregiver bills still overwhelm them.
ADU construction typically costs $100,000–$300,000, before architect, utility and permit expenses. CareScout found annual private-room nursing-home costs rose from $111,000 in 2022 to $130,000 in 2025, making an ADU’s upfront cost comparable to a year of care in some cases.
Families also described permitting difficulties and unexpected expenses, and a backyard unit may not meet rising care needs. Cornell researcher Mildred Warner said ADUs are useful when care needs are limited or a household member can provide more care. Alison George’s mother eventually moved to memory care and then a Medicaid facility as her Alzheimer’s worsened, leaving the guest house empty.
